Black holes are imploding stars, but they are also at the center of most galaxies including our own. Who knows there could be one at the center of the universe or we could be inside of one. Some have speculated that our universe could be a "hologram" on the surface of a black hole with all information encoded on its event horizen or "edge". Tiny worm holes are around us all the time everywhere with quarks zipping in and out.

Black holes are created by gravity.
To travel through a worm hole you need to become as skinny as a worm.

I believe God brought it all together to be out there is space for some purpose.
I think I've heard of the imploding star idea too.
Somehow things come completely apart while going through a black hole and come back together on the other side, where the 'traveler' also is suddenly then in a new world/time that exists on the other side of the black hole...it's 4th dimensional travel, where things happen in ways that are simply utterly impossible on Earth, humanly speaking concerning life as we know it in the third dimension.
Ever seen Through The Wormhole with Morgan Freeman on the Science Channel? Every episode is good- they delve into this subject and lots of others concerning black holes, the 4th dimension, etc.
I love Doc Brown's answer to Marty in Back to the Future when he questions how in the world it's going to work - "You're just not thinking fourth dimensionally!"

imploding stars create black holes... Imagine something the size of our sun, compressed into the size of a atom, it tears the very fabric of space time. (space time, look it up. www.dictionary.com)
No worm hole traveling would be improbable (not impossible per-say) because you as a person can't be in 2 separate locations at the same time...unless you understand some basic quantum mechanics / physics.
